I think I have seen this when the file manager (dynutil, maybe) had the 
file screwed up, two entries for the same file or something.  At any rate, 
if David W. doesn't see it in the code, you might (if feasible):

1)  Delete the file and make sure it is not in any VTOC.
2)  Recreate the file and run again

Hi,
    Can anyone help me with this?  A program reads the same extents 
repeateadly.  What could have caused this?
